Pacific Post Acute is the best nursing home in Santa Monica. I have seen all of them and Pacific Post Acute is the cleaniest, nicest, and has very friendly staff. Don�۪t think twice, you can trust this place.
I have been visiting Pacific Post Acute off and on for a few years. I have had friends and family members who have been patients there. The service rendered is world class. The nurses and staff are fantastic. They are very attentive to the needs of not just the patients but also the guests! Parking is a little rough at times, but that's how Santa Monica goes! I whole heartedly reccomend Pacific Post Acute to anybody looking to have their loved one get better and be able to go back home!
I had a chance to visit the place and Pacific Post acute employees are very professional and friendly. You can tell their staff cares about and interacts a lot with their patients.The overall atmosphere is very warm and caring, its definitely at great place for a loved one. 10/10

I really admire them for being more than willing to ask for help from other professionals in the field when they don't have an answer to my question. They aren't even slightly arrogant, and I'm confident they always have my best interest at heart. I was so impressed with the progress I was able to make after visiting them. All my questions were answered, and I feel a lot more confident and hopeful about the future. They went out of their way to make me feel special and respected, and were very sympathetic to my situation. I've never seen more kindness and empathy anywhere else. Tanya, the administrator, will do ANYTHING to make you and your loved one happy. One downside: The food is terrible.
In my job as a senior housing placement professional, I spend time in many skilled nursing facilities talking with patients and learning about their needs from staff. I highly recommend Pacific Post Acute. Pacific Post Acute is a smaller skilled nursing and rehab center which allows it to provide a level of individualized care and responsiveness often not seen at these types of communities. Located near Providence St Johns and UCLA Santa Monica, your loved one will be in a safe environment, clean, not depressing, and where he or she will receive the care needed to quickly rehabilitate.
As the husband of one of your patients who has been in this facility for six full weeks (April to May 2018) 12-14 hours a day, I have many good things to say. My wife got here without the ability to even sit on her bed by herself. It was even impossible for her to stand up even for a few seconds. With the expert care of the group of therapists led by Therapy Director Daniel Perischetti, my wife is now able to walk with a walker and almost ready to be discharged. Hats off to the other excellent therapists, namely: PTs Lisa and Carmen, OTs Tom and Liz, and even OT-trainee Sam. We all saw an improvement from day to day, and we cannot thank these therapists enough for preparing my wife to get herself reintegrated into the outside world. The nursing staff led by "Ate Mirla" were superb and dispatched their duties with a lot of loving care, respect and compassion. From my several hundred hours of observation, special mention belongs to the following: RNAs Salima (who also brings in a lot of fun and joy each time) and Rachmaninov, LVNs Eva, Ruth, Kevin, Cherry, and Garret, CNA's Eulalia, Ashley, Martha, Rosie, Mira, Francesca, Gloria, Isabel and other staff: Malou (Business Office), Lupita (Housekeeping), Jojo (Human Resources), Rosemary (Kitchen), Cindy (Social Services) and Marcela (Activities Director). Kudos to James (Administrator) for his management by example. BTW, for many years I was Vice President And Director for Human Resources and Executive Assessment Consultant, so I know what I'm talking about. I highly recommend this facility.

The staff and atmosphere at Pacific are very warm, welcoming and clean. Their team of professionals are both thoughtful and creative in shaping and maintaining their plans of care. I would feel confident in sending a close friend or loved one to Pacific Post Acute.
This is an amazing facility--from wound care (pressure ulcers) to physical and occupational therapy. The staff, especially the Director of Nursing (Liza Salcedio) and the physician in charge, Dr. Garcia, as well as the entire PT staff and nurses, have helped my elderly friend regain her physical and emotional strength. They care so deeply for all of their patients. The degree of caring and compassion is so impressive. We have used a number of rehab facilities in the area, and this one is, by far, the absolute best.
I visited Pacific Post-Acute to get familiar with the client needs and staff. I was wowed when I walked in! The staff at the front were very friendly and welcoming. As I toured the facility it was clean and staff were accommodating to the residents. Activities were going on and the residents were laughing and having a good time. As a Recreation Therapist, I like to see residents engaged and having a good time. I would definitely recommend Pacific Post-Acute!
What an awesome group of health care professionals! The CNA's,like Gloria and Abena, the physical therapists,occupational therapists, speech therapists, like Daniel, Tom, Elizabeth, the director James, the nurses like Kevin, Salima and Garett, the activities coordinator,Esperanza, the accountant,MaLou, the receptionist, Stephanie,the doctor assigned to the facility Dr. Garcia, were all caring, helpful individuals. Everyone there went out of their way to make my mother feel comfortable. They all watched out for her, treated her with kindness, love and respect. Would someone tell me why that god awful Berkely East where my mother was previously seems to be recommended???? This is my mother's 6 time in rehab over the last 10 years, so I know of what I speak. (She was previous to this year in the valley.)
I came to Pacific in very poor heath and left in very good health. The people are the reason. The care and concern on everyone's part was outstanding. I truly thought, I would go out in a box instead went out feeling on the TOP OF THE WORLD. Thank you everyone!

What a great facility! Pacific post acute is a small homelike skilled nursing facility where the staff members treat you like family! The rehab team is knowledgeable and encouraging, nurses are attentive and the management team genuinely care. The facility smells and feels clean.

My dad was admitted this past fall due to a foot injury. He is 81 and mentally with it! He appreciated everyone's assistance at the facility. Because his injury left him having to learn how to walk with a walker we appreciated the fact that the facility was all on street level. It was easy to get him in and out for doctor appointments. The staff were very helpful and were genuinely interested in making sure he had a good experience. They would even check on whether he liked a meal! They definitely tried!! He also had a very good experience with the PT team. He ended up leaving early because he was able to walk faster than expected. This was in part due to the excellent physical therapy regiment. I would recommend this facility.

I have been amazed by their attention to detail and the quality of care that they provide. They will go above and beyond to meet the needs of you and your family. I have been very impressed with the good people at Pacific. Both the administration and care staff are genuinely nice people who care about the residents and the care they receive. The facility has a very home-like feel in a residential setting, including a garden area in the center. The facility is always clean and welcoming. I would recommend it to my friends and family if they are in need of a rehab facility for themselves or their family members.
My mother was released on June 27th 2016 after spending 2 months under the care of a professional and caring staff, She was sad to leave and wishes she could live there permanently. Unfortunately she had to move to her new assistant living home. I am extremely happy with the care and attention my mother received at Pacific Post Acute center. I want to personally thank the activities coordinator Virginia and Dave Tanner along with the rest of the staff for taking care of my mother and making her time there pleasant.
I have had the pleasure of working in various facilities. All marvelous in their own way, coming into Pacific was by far a different experience. I made a home there with the residents and with the staff. Big shoutout to Tanya, she is an amazing human being always going above and beyond to help everyone and ensuring the best stay possible. Don't want to take my word for it, you don't have to. Go visit! You will definitely see it for yourself and you will not be disappointed with the people you encounter.
My family and I have had a wonderful experience . The staff, the rehab, and the facility are all first rate. It is not fancy and a little dated but is always clean and has a nice courtyard. The care is excellent and the staff is friendly, attentive and helpful. Would definitely return if necessary and recommend to others
I would give Pacific Convalescent Center an excellent rating because of it's warm, friendly and competent staff. The place is always clean and there are a lot of activities on a day to day basis for it's patients. Kudos to you all!
I don't usually post, but I felt compelled to do so as this is a very important subject when it comes to our loved ones. My mother lived with me for 10 years until she got to a point that she needed more medical care than I could provide her at home. I never wanted her to go to a nursing home but I did not have a choice. I am a big researcher on any subject, and when it came to my mother, my best friend, I spent endless hours driving all over the city going to visit different places. I must have gone to at least 15 facilities, each one was worse than the other. I then went to Pacific Post-Acute and I was so impressed with how clean and nice it was. I couldn't beleive it. Their staff was so nice and friendly. Their director, Tanya Papazian, made the entire process so nice, she was very informative, and she helped me with everything. She works very hard to make sure all her residents have what they need and her staff clearly loves her. The rehab helped my mother. I went to visit my mother 7 days a week for an entire year, until she passed. If anyone can testify to the hospitable, cleanliness, rehabilitation qualities of this place, is me. I recommend this place to anyone who needs to be in this type of facility for any reason, you won't find better.
As a professional in Senior Care, it is very important for me to know how the patients are been taking care of while transitioning at a Rehabilitation Setting or post Acute for that matter. Some of my Residents have been admitted at Pacific post-acute in the past and every single time Pacific has deliver the best care and timely rehabilitation to each one of them. I won't hesitate to recommend Pacific-Posta acute to anyone that is in need of a good rehabilitation center.
As far as nursing homes, I would honestly say this is probably the best I've found. The staff and administration is spot on, genuinely hospitable all around, the rehab is for sure 5 stars. If you're looking for a spot for a family member, definitely consider it, you won't be sorry. I've done my research!!! Trust me
I'm so glad I picked this facility! The rehab team here really knows what they are doing and are true all-stars! The place is a little small compared to most but really has a warm feeling to it. Loved that it had a nice outside patio and was especially impressed there was no bad smell in the building. I got to personally know many of the nurses who I felt did a great job with the nursing care. Awesome team and place!
As per the other yelp reviewers, my husband and I also found Pacific to be extremely top-notch! On first entering the facility one notices the pleasant fresh smell (unfortunately a rarity in some other nursing facilities...), the abundance of lovely light, and an overall upbeat and pleasing atmosphere. The outstanding amenities include very large rooms, a gym/physical activity program, physical therapy, and an enclosed garden where residents are even encouraged to participate in gardening. David provided us with a very comprehensive tour and Esperanza the activities director seems perfectly suited for her position. As mentioned in the other reviews, the staff (amongst whom we also met Brian an Tanya)is cheerful and welcoming. If your circumstance places you in need of this type of care, you couldn't do much better than Pacific
My mother was here for almost 3 weeks following a broken pelvis and the staff were outstanding. The PT and OT are done almost every day and my mother made great progress. They were outstanding. The director Janes Mackey was terrific - working with us to get the right set up for my mother. The setting is not fancy and residents mixed so not necessarily a comfortable setting for folks used to a more upscale environment but the care made it worth it. For few nights we hired a private caregiver to be sure my mother had constant attention and the staff were more than willing to work with us for that which was very appreciated. UCLA geriatricians also have privileges there and are there a few times a week which is also very helpful. So great for post-injury rehab for elderly!
After a total knee replacement surgery, I needed to move to a facility with a first rate rehab. team. Pacific Post-Acute met the requirement and provided excellent care. The physical and occupational therapy staff (Daniel, Tom and Carmen) were outstanding. I am sure that my rapid recovery is due to their excellent work. The entire staff at Pacific is professional, friendly and made my 2 week stay very positive.
Pacific Post-Acute is a great skilled nursing facility. They provide an outstanding environment for recovery and rehabilitation with their amazing nurses, physical therapists, physicians, and administrative staff. I was so impressed by the quality of care provided by the care team, their friendliness, and their dedication to their patients. One of the best skilled nursing facilities I have seen in the Santa Monica area with a care team I trust with my loved ones.

My sister in law discharged from Pacific Post Acute earlier today after being there for about 3 weeks for rehab. She was a bit scared to leave because of what a great job they did. The therapy and nursing team worked miracles with her and we all feel like she�۪s now ready to come home. They gave her the tools to be successful and even helped train us a bit to make sure we can help. The staff was always attentive to her needs, even while busy. She was there during a transition with administrative staff, and she fell in love with the new guy because of how friendly he was in making sure she was ok and happy.

My experience with my mother's care at this facility has been top notch. All the staff are attentive and caring and go above and beyond with the care they provide. Director James along with manager Tonya and nurses: Gloria, Martha, Kevin, and Salima, OT: Carmen and Tom have been amazing in helping support my mom through this journey. Rachna, my main squeeze, exceeded my expectations to make sure my mom was comfortable and kept her in good spirits through her rehabilitation. All in all, the entire staff has been great and I would recommend this facility to anyone needing care for their loved ones.
My aged and terminally ill family member was cared for at Pacific Post-Acute, and my conclusion is that they provide a level of care that's the next best thing to having medical equipment at home and one's own family trained as healthcare professionals looking after your loved one. Pacific's professional management and staff truly care that much. These are wonderful and dedicated people who deserve our trust and gratitude for what they've freely chosen to do. We're lucky to have them.
