My 85-year old sister-in-law has been in this facility for over three years. She has some health problems as well as some cognitive issues. We cannot say enough positives about this facility. We have communicated primarily with Clarabel, who is with Social Services at this facility. Communication is timely and ongoing. The staff is exceptionally welcoming, warm and friendly. Sally, the activities director, provides an active program for the residents, and my sister-in-law is an active participant in the many choices of activities that are provided. We have been extremely pleased with the care that has been provided.
We visited several facilities before decided upon Garden Park, and I'm glad we did. The nursing staff took great care of my brother during his last few months. The only complaint my brother ever had was the food, but then again, what can one expect of hospital food They kept us informed of his progress and condition. The facility is clean and well-staffed. The employees are well-trained and very compassionate. Thank you, Garden Park nursing staff, for making my brother's last few months pleasant and comfortable.

My father (Alexander) came to the Garden Park Care Center from the Garden Grove Hospital for rehab and he became a long-term patient at the facility. It's always disconcerting to say the least to have heard so many negative stories about nursing homes in regards to their facility and staff. But here's our family's experience with Garden Park Care Center. After a detailed tour from Faby, the Admission Director, my dad went came as a short-term patient. She was so welcoming, detailed and attentive during the tour. I didn't feel like she was trying to sell me a place for my dad. Since then he's become to long-term patient under the care of great nurses like MaryAnn and CNA's, Anna. What I've noticed since then is the collaboration that occurs amongst the staff: Mr. Clayton in admin, nurses, social services, public relations, maintenance, CNA's, front desk and other staff that work behind the scene. They work as a team to manage the facility and care of their patients and it's evident. (Sure there are always things to improve upon, as is true of any business), however, I want to share the big picture of this site. I've come various times of the day to visit my dad all these months and the facility is well taken care of, patients are well groo, safe and given attention when they need it. Staff members are productive and ready to assist when I've had questions or needed clarification, like Clarabel and Lyly in Social Services, Theresa in Billing, Faby in Admissions, Lily as the RN, Ana as my dad's current CNA, Elizabeth and Sally in Public Relations and the PT department. There are many others I want to thank for the caring of my dad while he's at Garden Park. Our family is happy my dad is in good hands and appreciate the positive and hard work on everyone's part.
Really great place due to that it's very quiet. Friendly environment there and staff as well. I've done my eagle project on 8/15/2015 on building benches. Craig and the staff was really nice to provide that opportunity to myself and my fellow people in the troop. I would definitely would like to come back again.
Well maintained facility and an amazingly courteous and helpful staff on hand. Been here since 7/27/16 and have not had a single bad experience. Wide variety on daily menu and food is well above average.
I have been here at Garden Park rehab center for about a month now. When I first arrived here I was unable to walk, etc. and now, I am walking and almost 100% again and going home this coming Monday! I want to thank the whole staff (which there are too many to name), but they are all so caring and compassionate. They have helped me through one of the worst times in my life and I would highly recommend this place to anyone. Douglas Richey
This is my 15th orthopedic surgery i have had in 5yrs. All of my surgeries I have been able 2 go home the same day. My doctor told me this procedure was going 2 b more painful then the 1's my orthopedic Dr. Lauchlan Chambers had preformed. He was right. I couldn't walk or stand when I woke up from surgery due 2 severe pain. I was admitted 2 Fountain Valley Hospital 3 days. On the 3rd day Dr. Hilton said I couldn't just stay in the hospital but couldn't go home as well. He suggested a rehabilitation center. I agreed. Phone calls were made but no beds were available and 2 places said I was 2 young which upset me. Stayed the night. On the 4th day he came in 2 say he had found a place that would take me: Garden Park Rehabilitation Center. I was very excited. I arrived at night via ambulance with my mom inside. Once in my room I was greeted by Elizabeth, Craig, and 2 nurses. They could tell I was nervous, in pain and hungry. I had never been 2 a place like this b4. Elizabeth disappeared 4 a few min and came back with this really neat basket arrangement like the kind u would mail 2 a friend 4 a special occasion. Inside there was a green plastic basket filled with hygiene products and things I would b needing during my stay which was perfect since I didn't have anything with me. Elizabeth went over my daily schedule. After lunch many people came 2 c me 2 get personal info. I also met my day nurse who was totally awesome!! I liked her a lot. Then came the physical therapist. The person who I was dreading the most. I had 2 use a walker which I hated. Felt like 4 ever getting 2 the physical therapy place. Everyone in there greeted me with a smile. At that point of my stay I couldn't do anything 4 myself. I mean literally. Maybe I was taken in a wheelchair. She just asked me questions and had me do a few things. Taught me how 2 use the bathroom using a shower chair over the toilet. Also taught me how 2 get in and out of bed. Each day I got stronger and was able 2 do more things 4 myself. I was able 2 take my pain meds every 4hrs. I asked my day nurse if she could wash my hair. She said yes. I was concerned about having 2 get into the shower area and how was she going 2 make sure my bandage didn't get wet. She put towels around me then lowered the bed. My hair was being washed in bed!!!! I thought that was so amazing!!! I would of never thought of that if my life depended on it. Whoever came up with that idea is brilliant!! I had a lot more physical therapy which made me stronger. By the 5th day I was ready 2 go home. I could walk with my crutches, use a normal toilet, get in and out of bed, brush my teeth and so on. Day 6 is when I got discharged. I was beyond happy 2 go home 2 sleep in my own bed with my own blankets, pillow and my Angry Birds stuffies. BUT I was sad 2 leave the staff who took great care of me. I took photos with the staff members who came in 2 say goodby. Elizabeth handed me a card which had a lot of goodby messages in it with very kind words. I highly recommend this facility. I'm coming back when I have my left hip and final surgery.

I had a hip replacement surgery and was sent to Garden Park for some recovery/rehab. Was there for about 10 days and released today. I've never been in a hospital situation for this long and figured I'd be going insane. Well, while I definitely did miss home and my own bed etc, I must say the staff here were so warm and caring that it made it so much easier being away from my own environment. One person that I won't forget is Estella. She has got to be the sweetest most caring and down to earth lady there. They're very lucky to have such a wonderful person and I can't say enough good things about her. Everyone there was great! ALL the staff at garden Park Care Center are AMAZING!!! It is hard for families to deal with aging family members but having a place like GPCC for my mom to be in makes me more comfortable. She was there at the facility for 10 months and I have the utmost respect and gratitude for everyone there who helped my mom.
Staff here is great. Welcoming and friendly. And the facility is CLEAN, with a great sub acute department.
My Aunt is in there and they give her very good care. She is very happy there. The nurses are always checking in on her and they are very friendly.
I was a resident at this facility 1 year in the Subacute unit & 1 year in the SNF unit. the nurses, care givers were very good to me sometimes I was cranky because of my pain and they were patient and caring. I recommend this facility to anyone that is looking for a rehabilitation facility. activities really stood out it was instrumental to my recovery.
Garden Park Care Center provides quality care to my grandfather. I come here often to visit him and the staff is always friendly and seems to try their hardest to care for their visitors and residents. I feel comfortable with having my grandfather staying here because he seems to be in good spirits as a result of being in good care.

All of the doctors told me that there was no hope for him to ever improve. However, thanks to the good quality care that he has received over the last 8 years, he and I can communicate with each other. And I have faith in the Lord Jesus Christ that someday he will be able to return home to live with me in perfect condition. All of the staff members at Garden Park are very friendly and professional. I am greeted by name every day when I arrive. Everyone treat me like family so I feel very comfortable here. I live far away but choose to keep him here because I am so pleased with the care that he receives. My husband is always clean and freshly shaven when I arrive each day. The nursing team and CNAs make sure that his every need is taken care of. Although his skin has become thin and sensitive with age, they manage to keep his skin healthy and free of sores. Because the employees are so trustworthy, I never have to worry about his being well cared for in my absence. I appreciate that the facility is always clean and sparkling, and never smells. There is a lot of visitor parking so I always find a parking place in front. There is a park with a lake next door that I enjoy walking around every day as well. I surely recommend Garden Park Care Center to anyone who has a loved one in need of skilled nursing care. I am extremely satisfied with every aspect of his care and grateful for all of the love and care he receives.

I can't say enough good things about Garden Park. My brother has a traumatic brain injury. At first he was at another facility that didn't care for him correctly, and my family had to be on the phone with them several times a day to make sure he was cared for. Lucky for him he was taken out of there since he was near death and about 100 pounds since the staff would just leave his food in front of him and leave, when his file clearly said he couldn't eat on his own. He checked into the hospital and had brain surgery, then orange global hospital wanted to ship him to any place that had a bed. This is how he was put into the last place, so this time we demanded they keep him while we find a place that will really care for him. After searching long and hard we decided Garden Park sounded like a great match. We called and talked to Faby who is so nice and helpful. We told her his situation and she helped make sure that when a bed was available that he'd be brought in. Of course it was a holiday weekend so we were on pins and needles with the hospital wanting him out. We knew that we had to get him in here so we didn't budge. After the holiday we got the ok and we were told that he was being sent there. Our family couldn't be happier. Of course we were still scared because the last place had us weary. Well from the second he got there the staff has been a dream! They literally exceeded our expectations by far! When you walk in everyone is super nice and talkative (unlike the other place that won't stop when you ask something, or would act like you're putting them out over important questions).They treat him not only like a human being, but like a friend and are so caring and gentle. They have him out eating with other residents and doing activities during the day. This place really cares about the residents and you can feel it. If I could give this place 100 stars I would! With a brain injury its so important he get consistent therapy and care to get back to the person he was before the injury. My brother has a real chance at full recovery now because of Garden Park. I highly recommend this place. :)
My Mother was transferred to Garden Park Care Center from UCI Medical Center for physical therapy. During two months stay, my Mother is very happy with the services provided by this nursing facility. The place is always clean and organized. The staff is very friendly, professional, helpful, and responsible. Special "Thank You" to Dr. Dat Nguyen, Madel (Case Manager), Elizabeth Pham, and Fabi in the front office. People in Physical Team. Mindy, Nancy, Carolyn, and Assistants in Nurse Station. And people who I cannot remember the name. I would recommend Garden Park Care Center to everyone who needs the assisted living.
This is the first time in my life that I had to experience a Care Center for myself. The staff here at Garden Centre were welcoming caring friendly and just made me feel special what a wonderful feeling that is when you're hurting. I would recommend this center to any of my friends or family that needed care and if ever I need convalescent care again I will definitely come here.

My daddy has been rehabing here for going on 4 months now. The facility is great. They are a first rate staff , always willing to answer questions and dad's well being and getting home are the first and only goals. They have been attentive to all his needs and are ever trying to lift his spirits to motivate him. I would not hesitate if my loved one needed extra recoup time to stay here. Even the longer drive hss been worth the great staff and care he is getting. Thanks everyone ! Not going to stay a day longer than necessary but he's so much better eveyday because of all of your hard work and care.
I am actually a physician who works independently of this facility. Over the past two years I have referred some patients to this center for subacute and skilled nursing care. It is important to note that I had been to this facility several years ago; just one visit to see a patient who I discharged from the hospital for a social visit. At that time I was very disappointed. Since then, the ownership has apparently changed and quality has picked up considerably. Since then, I learned that another patient I cared for was transferred there for rehab care. The family was very apprehensive about going, so I offered to visit the patient and keep an eye on things so that they would feel better about the transition. Compared to my first encounter, my second experience was considerably much better. I met with the administration and learned about the changes they had made to improve the facility. I get the sense that they try really hard to accommodate their patients individual needs. Currently, I have some patients in the sub acute side who are on ventilators, and a couple who are on the skilled nursing side. When I ask for specifics, the nurses are very quick to respond, and they follow through on getting my patients the specific care they need. Of the very few patients I have cared for there, I have a number of success stories of patients I still keep in touch with. My advice would be to look at the facility (any facility) with an open mind, and be involved in the progress with any patient's care. Don't hesitate to ask questions, and then you can judge for yourself whether the facility meets or exceeds your expectations.
Couldn't have asked for anyone better to make this hard decision easier on our family. Lani was very kind and knowledgable about her facility. She didn't see us just as customers she treated us more as family. I knew my family member would be in great hands if the rest of the staff was the same as Lani was. My intuition was correct, from the physical therapists, nurses to facility representatives carried themselves very professional. Knowing there are many facilities around the area to choose from for your loved ones, I would not hesitate recommending you Garden Park and don't forget to ask for Lani.
The management and employees are very friendly, attentive and make you feel welcome every time I visit my relative at Garden Park Care Center. The comfort and care she receives is more than I ever expected. They are always aware of the patients needs and check on them regularly. The facility is older but surprisingly well maintained. I would not hesitate to recommend Garden Park Care Center to everyone.

I am currently working as an EMT and I often come here while at work. This facility is one of the cleanest facilities that I've been too. They also have a very friendly and knowledgable staff that makes the difference. The facility is really nice and in a nice quiet environment.

My father, Thaddeus Pham, spent his final months at Garden Park and the staff was so supportive to my family and I during this difficult time. I could always count on his favorite CNA, Jorge Medina, to take the best care of him. Jorge was so kind and treated my father like a loved one. To this day I still stop by Garden Park to thank all the staff who treat my father with respect and care. I am forever grateful for the kindness they showed to my father and family. Lastly, Garden Park cares about its patients and it shows. Since my father's passing, they have a dedicated staff member whose sole responsibility is to advocate for patients/families and their issues. Now THAT is customer-centric. Keep up the amazing work, Garden Park and continue to put patients and their families first by having the best staff there is.

My father stayed at this facility for almost 3 weeks. The staffs and nurses, PA, physical therapy..etc.. were exceptional: friendly, caring, professional, empathy. We, as family members, highly appreciated all the help and care from everybody at the center, from the director to all the nurses. Highly recommended!! Thank you all..

Wide variety on daily menu and food is well above average. The Wound Care staff and visiting Doctor did a GREAT job with regards to treatment of my leg wounds. I've healed completely and will be discharged tomorrow. I'll miss the friendly staff who've become my secondary family during my stay.
This place is amazing! Our representative for care is Lani Phan. She's a hard working woman and very friendly! The facility was everything she promised from care to cleanliness!
My mom has just joined the family at Garden Park Care, she is happy I can tell, the staff is friendly and very accommodating. I had her in a place in Santa Ana, it was THE most depressing place dark and dreary and it stunk most of the time. Garden Park is a breath of fresh air (literally). I am very happy with this facility and feel at ease about Mom being there, she will be well taken care of.. I would absolutely recommend it to friends or family. Thank You all.
7 years of being in the medical field and working with skilled nursing facilities and never have a seen the level of management here. They are very on top of things. My aunt spent a month here and the staff was very caring and compassionate as well as competent. The care manager Madell answered all of my questions. They made sure my aunt had everything she needed. The PT made sure to get her out of bed. I was very impressed with the follow up for doctors appointments and discharge planning. Would highly recommend this place for your loved ones.
2/08/2016 My dad is a 94 yo retired navy veteran. He had a stroke about 10 years ago, fully recovered. Lately he has grown physically weak and frail. His heart and vitals are strong though. He was referred here by fountain valley community hospital. Hes been here at Garden Park for 2 1/2 months. My mom was admitted to a nursing home when she was alive, the service was terrible. We ended up taking her home so we could care for her better. Our experience with our mom made us dread the fact that my dad would be coming to one these places. However, to our surprise the staff were friendly, helpful and responsive. They changed his pamper every 2-3 hrs. administed his meds daily. He also does physical therapy and has a speech therapist. Hes connected to a feeding tube now, but the food was decent when he was able to eat on his own. I would recommend this place to anyone who needs assisted living. It makes taking care of our loved ones alot easier. The facility is old but the service is awesome. Its so sad seeing some of these patients living out their last days. Each of of them has beautiful life story. Many get dropped off and are abandoned by their families. Exactly like they do at an animal shelter. I want to commend the staff here for running a tight ship and for the unappreciated service they do for our loved ones.

They took extremely good care of my grandmother and made sure she was comfortable. She is a very hard to please woman and every time I came to visit her she was all smiles. I am/was a resident of this facility i was a resadent at Garden Park for a year. That's how long it took me to get way better. i loved the nurses, they give you hugs! And they really mean it. Now im on my own and i miss my girls very much. When you go to a new place you will notice the place is not what your use to. I still have found memories of Garden Park. It's a great place with staff with experience and knowledge on how to handle elderly patients and as a fact, my father is a patient on this nurturing center. I would recommend this facility to any elders who want quality treatment and care. Hopefully, this review has influenced others to come to this place.
I deliver food for a patient and the place is absolutely clean. Staff are nice and helpful and parking is plenty. Restrooms are never dirty and the patients look content and happy. This care Center provides a great comfort to those in need.

I walked into Garden Park Care Center and was greeted warmly by Angie. Everything smelled great and there was a sense of "welcome" when I walked through the door. Sometimes you don't remember how things looked or what was said...you always remember how a place or person makes you feel (Maya Angelou), I felt very welcome here and had a great sense of professionalism.
Easy place to find. Great parking. When I step in, I felt like I'm home. Hand sanitizer in the front is great and useful. Workers are very nice.10/10 would come again!
Been volunteering here for many years The staff is friendly and they treat the patients well, the location is older but they have remodel the building during the past few years, this place have activities during Saturday and Sunday. They always allow visitors 8-5pm and even late at night, very cool location no too strick, would recommend to many of friends

Definitely 100% better than the previous nursing home my grandma had before. Everyone welcomed us and made us feel that my grandma will be well taken care of. I love how the physical therapists are very involved with her plan of care (which btw is the first time for her. on her previous nsg home,they barely even perform passive ROM on her). Shout out to Debra and Hector for being the best PTs ever!My family and I are very thankful since they took initiative to sit my grandma up!Love them!The nurses are all good too!i have trouble remembering names but yes,everyone's very attentive and accomodating. We were not sure about transferring her here at first because it is quite far from where we live but the travel is definitely worth it! Keep up the great work guys!
